Have you looked in the radiator to see if there is oil mixed in with the coolant, or pulled out the dipstick to see if there is water in the oil? if I had to guess based on what you have described I would say is sounds like a blown head gasket. I would suggest doing a compression test as a first step.

I think fasteddie may be right. You should check to see if the motor mounts have collapsed. This can allow the engine to sit a few inches lower and even move forwards and back. It is strange that we all have the same Isuzu 4-speed manual transmission but mine was put in a GMC pickup and the users manual and the shop manual say to use ATF in the transmission even though it is manual. I have owned the truck since new and that is all I have ever used and I now have 417k miles.
